Borrower’s Agreement
Borrowing Privileges – Individuals may borrow books/materials by signing the Borrower’s agreement.
A Borrower’s agreement is issued initially for three months and may be renewed as long as no outstanding obligations exist.
Requests for renewal of this card may be made through the mail, e-mail or in person.
Loan Period – All materials have a loan period of 1 month.
The borrower’s agreement is nontransferable and must be accompanied by a current, government-issued picture ID – such as (but not limited to) a driver’s license, or ID card, US passport, or US military ID – to borrow items.
Borrowers can check out a maximum of 3 books at any one time.
As long as a book has not been reserved, patrons may renew their books twice.
Reserved books cannot be renewed.
The borrower is responsible for all items checked out in his/her name until those items are returned and cleared from their library account.
Fines – Books are due on the date that appears on the due date slip located in the book.
Fines begin on the seventh (7th) day an item is overdue. Overdue fines are $.50 per day, with fines not to exceed the total value of the book/materials.
Fines and payments: can be paid in person, with cash.
Payments can also be made by mail with a check or money order made payable to the Twin State Depression Support Group, PO Box 27, Ascutney, VT 05030-0027
Notices – Overdue notices, recall notices and fine bills are sent by email.
These notices are sent as a courtesy; the Twin State Depression Support Group is not responsible if a patron fails to receive a notice.
The patron is responsible for updating their address/email change immediately.
Lost and Damaged Items – A patron who suspects that an item checked out in his/her name has been lost he/she should report it as soon as possible by emailing TwinStateDSG@gmail.com or calling 802-795-0098.
A patron who loses an item is responsible for reimbursing the Twin State Depression Support Group for the replacement cost, the processing fee, and any/all fines accrued.
Patrons are responsible for the proper care of items that they have checked out.
The cost of repair or replacement of damaged items will be billed to the borrower.
